Early 2008 Mac Pro - 4th time is the charm!
 

FYI, I was frustrated that my MacPro wouldn't even show the adapter in System Profiler, but I unplugged stuff out of several USB ports and found that on the 4th port I tried, the computer recognized it and it's up and running and doing fine.
