Sadly, I don't even see USB enumeration of the device in the logs you provided.
You should start troubleshooting from the device side - is it a power adapter? a broken USB cable? a dock itself? Thanks Michal |

An Update...
I went ahead and obtained a Diamond USB Display Adaptor Pro (USB 2.0 to VGA/DVI/HDMI) adapter. It arrived just a little bit ago, I hooked it to my DVI cable and I'm pleased to say that, for now, the third monitor is back in action.
