So this issue has come back and what originally...
So this issue has come back and what originally worked above no longer works.
Third monitor no longer comes on and I keep seeing the same messages in log.
The only thing I can think of at this...

This adapter originally worked fine on OSX 10.7.5, then after updating to 10.8.0 it stopped working in July 2012.
I have since reformatted with a fresh 10.8.2 and tried the new 2.0 alpha driver...
