Are you using a USB Y-cable with this product?
If not, do you have an externally powered USB hub?
From the description, it sounds like a brown-out issue. More power needed than a single port can deliver, so voltage drops, the screen switches off before starting again...
The fact that the device drops from the USB, and Device Manager rules out a driver problem: before Windows knows which drivers to assign to a device, it needs to see that device.
