Just to re-cap, here's my config:
left monitor 1920x1200, using DVD-D to DVD-D cable from laptop dock
center monitor 2560x1440 using DisplayPort to DisplayPort cable from laptop
right monitor using Lenovo USB 3.0 dock DVD-D to HDMI 1920x1080

Here's what did work for me:

Used the new DisplayLink USB Graphics Software for Windows* V7.0 M2

And tada, it updated the firmware in the Lenovo dock, and suddenly my 3rd display began working again.

Yep, the fix was that easy for me.

I also noticed that it only works when my ThinkPad W520 is set to Discreet Graphics mode in the BIOS.

So I made sure I'm using all the latest Windows 8 drivers, which are now GA, here:

which means NVIDIA video driver 9.18.13.0614 / 9.17.10.2843, but it didn't matter, only Discreet Graphics seems to work with my config, Optimus mode is a no go (only my left and center monitors seen).

Annoying this in the Discreet Graphics mode, when my laptop is using its internal screen only, it locks up solid soon after finishing booting Windows 8. This was a problem throughout the betas, and now with the GA Win 8 64 bit and the GA level NVIDIA drivers as well, sigh. But that's a laptop issue, not a DisplayLink issue.

Anyhow, this is progress, turns out I didn't have to use the DisplayLink Install Cleaner.
