Default turn on debug mode, and it works!

I went into the debug tool, turned on debug verbosity, unplugged, plugged, and it now works!

I have included the zip

The only thing I did differently was to unplug the mini usb from the device, not from the computer. Perhaps a poor connection?

Default unattached, reattached 3 times, then it works

Can you verify that this is a faulty device before I RMA it with newegg.

It is:
SIIG USB 2.0 to DVI/VGA Pro JU-DV0112-S1 USB to DVI Interface
